The Housing Authority of the City of Santa Barbara is launching The Health Access and Care Coordination Project, an exciting new multi-agency collaborative to improve access to health care services for HACSB residents with MediCal ages 19-65!
Health Services Coordinator, Hannah Greenberg, is now available to help you connect with the HACC Project services and health activities.
The HACC Project offers:
- MediCal and Covered California health insurance enrollment assistance.
- Health promotion and wellness activities: FREE flu shots, diabetes screenings, Yoga classes, walks with a doctor or nurse at “Happy Feet Walking Club,” Health Fairs, nutrition classes and much more!
- Assistance with health care referrals, scheduling medical appointments, transportation to appointments, and follow-up calls.
- Home visits and “pop-up” clinics for physical and mental health assessments, and preliminary treatment.
